Getting started with IBH is easy

Complete the Right Form
- Psychotherapy or Medication Management — download and complete the Intake Form
- Psychiatric Rehabilitation Program (PRP), IOP-SUD, or Partial Hospitalization Program (PHP) — download and complete the IBH Referral Form
Submit Your Form — or Call to Schedule
- Email it in: send your completed Intake Form and/or IBH Referral Form to info@integratedbh.com
- Call to schedule: call us at (301) 499-4240 to book your first appointment, and complete your Intake Form and/or IBH Referral Form in person
Either way, your insurance card is needed to secure your appointment — see Step 3 below.
Provide Insurance Information (if applicable)
If you’re using insurance to cover your care, please email a copy of your insurance card (front and back) to info@integratedbh.com so we can verify your benefits before your visit.
Please note: if we book your appointment and haven’t received your insurance card, your appointment may be cancelled.
Attend Your First Appointment
Once your paperwork is received, our team will reach out to confirm your appointment. Your first visit is a chance to share your story and begin building a personalized care plan with a provider who’s the right fit for you.
We accept most major plans
IBH accepts most commercial insurance carriers, as well as Maryland Medicaid, Medicare, EAP, and cash pay. Our team is happy to help verify your coverage before your first appointment. Visit our Insurance page for full plan details.
